Catholic Charities Free Health Care Center provides quality medical and dental care to individuals who are 19 years of age or older and who live at or below 250% of the Federal Poverty Guideline. Many of our patients also face social, emotional, and/or cognitive issues. We serve the uninsured, underinsured, and underserved. We believe if we provide high-quality, free medical and dental care and if our patients take ownership of their own mental, physical, and emotional health, together we will enhance their individual lives and the quality of life among vulnerable populations. The impact in our community and to those we serve is four-fold. Our services 1. reduce out-of-pocket costs for patients; 2. address health issues prior to major deterioration; 3. emphasize preventive health maintenance; and 4. offer referrals for health care navigation for medical assistance and insurance/marketplace plans if determined ineligible for our Free Health Care Center services. The Catholic Charities Free Health Care Center is a Volunteers in Medicine, or VIM facility, utilizing the talents of active and retired dental, medical, and nonclinical volunteers supported by a core paid staff. Patients are seen on an appointment-only basis and have access to the following routine and preventative care services: * Primary care visits, including extensive first visit evaluations * Treatment plan development * Dental and health screenings * Dental and medical therapy and management * Chronic disease management * Coordination of preventative care * Health promotion and education, including diabetes education * Nutrition and diet counseling * Specialty care as provider availability allows (Medical specialties include: audiology; cardiology; dermatology; ear, nose, & throat; endocrinology; gynecology; ophthalmology; orthopedics; physical therapy; pulmonology; and rheumatology. Dental specialties include endodontic; prostheses/crowns; and periodontics.) * Care Coordination for any external provider referrals (private practices, medical/dental schools, etc.) * Coordination of financial assistance enrollment applications for any external lab and testing referrals and pharmacy assistance programs
Patients are also assessed for emotional well-being, food security, and other social determinants. Those who could benefit from additional assistance are referred to
the Catholic Charities case management team to position them on a path of lasting solutions.
